The world's exploding population signals even more growing pains ahead for already crowded
areas. A new United Nations study forecasts that by the year 2000,2 billion persons will be added to the 4.4 billion in the world today.

Even more troubling than the increasing number of inhabitants are the projections of where they will be concentrated. The study by Rafael M. Solos, executive director of the U. N. Fund for Population Activities, notes that by the year 2000:

Nearly 80 percent of all people will live in less developed countries, many hard pressed to support their present populations. That compares with 70 percent today.

In many of these Third World lands metropolises (大城市) will become centers of concentrated urban poverty because of a flood of migration from rural areas.

Speech therapists divide their cases into common and complex. An example of a complex case
is a person suffering from aphasia.This is the loss of the ability to under- stand and/or use speech. There are many causes of aphasia, including partial brain dam- age that leaves the individual with the physical ability to speak. If the individual is able to speak but does not, the therapist must determine the cause of such conduct. Often, speech therapists determine that problems are emotional and refer the individuals with such problems to psychologists or psychiatrists trained to help them.

While a speech therapist is not trained to help persons with severe emotional disorders, the therapist does receive extensive training in such areas as anatomy(as it relates to speech, concentrating on the diaphragm, tongue, and palate), physics(particularly relating to sound), and language and communications. Some therapists take classes in elementary psychology as well.

The speech therapist must be able to diagnose the type and severity of the speech defect, then prescribe a series of treatments. While some people with severe handicaps receive individual instruction, most patients are comfortable receiving group therapy with others who have the same problem. If a patient is found to have a physical handicap that is impeding his progress, he is sent to a doctor with whom the therapist works closely.

Many therapists have a high success rate with their patients. No scientific studies have yet shown whether the intervention of the therapist was in fact the sole cause of the improvement of the patients, or whether time and maturity helped as well. A recent study has shown that persons who have not received speech therapy have often been "cured' of their problems merely by being around others without the problem. However, that same study showed that those few persons who were not treated by therapists had extremely low self-esteem and considered themselves " different" and "retarded". The therapists often are able to make the patients see that while they are different, the difference is not of their own making, that they have not done something wrong. Therapists try to reach children when they are very young, before they have had an opportunity to be teased by other children or made to feel they are "stupid". Therapists often work with whole families, encouraging the parents and siblings to listen to the patients, taking the time to help them feel they are an important part of the family, not just brushing them off because the effort of interpreting their speech is too great or time-consuming.

Recently housing is recognized as a "socially determinant variable". In France, housing is
the main item of expenditure in the family budget (accounting for an average of 29 percent), and many families would be unable to find decent housing without help from the State. For a long time, the main problem was the housing shortage but in recent years the deterioration of housing conditions has been giving even greater cause for concern.

Despite extensive construction programs, the problem of housing for the most underprivileged population groups has not been solved. According to the so-called Petrequin Report, between 2 and 3 million families had serious difficulties meeting their housing costs and were living in precarious and uncomfortable conditions.

Policies designed to address the housing problem have shifted over the past few decades from a macroeconomic approach promoting construction to housing subsidies. The reasons for this shift agree with the determination to limit public spending and to avoid some of the perverse effects of macroeconomic policies. The State has to some extent ceased to finance housing, especially the construction of new projects, with the result that the cost is now chiefly and directly borne by the family budget. Many underprivileged families, which were excluded from low-rent housing for various reasons (selection of tenants, saturation of existing capacity, insolvency), had no alternative but to purchase their own home and were encouraged to do so without restraint by the easy terms of housing loans. The housing sector thus contributed to the development of the "economy of indebtedness". It should indeed be emphasized that "widespread home ownership through resource to borrowing could only be to the detriment of low-income families".

In Belgium, the quality of housing, considered the prime indicator of housing deprivation, heaves much to be desired. Low-rent housing projects have been cut back as part of the austerity policy pursued by the national and regional governments, and low-income households are finding it increasingly difficult to find somewhere to live. The number of homeless has also taken on alarming proportions. An estimated 3,000 persons spend the night in refuges, but the figure is probably much higher. Moreover, the number of homeless women and young persons is increasing.
