The Issue of Package

Maybe everyone has such an experience that you have to unwrap several layers of packaging when you enjoy a piece of candy. But this overuse of wrapping is not confined to luxuries. It is becoming increasingly difficult to buy anything that is not done up in beautiful wrapping.

The package itself is of no interest to the shopper, who usually throws it away immediately. So why is it done? Some of it, like the cellophane on meat, is necessary, but most of the rest is simply competitive selling. This is absurd. Packaging is using up scarce energy and resources and messing up the environment.

Recycling is already happening with milk bottles which are returned to the dairies, washed out,and refilled. But both glass and paper are being threatened by the growing use of plastic. More dairies are experimenting with plastic bottles.

The trouble with plastic is that it does not rot. Some environmentalists argue that the only solution to the problem of ever increasing plastic containers is to do away with plastic altogether in the shops, a suggestion unacceptable to many manufacturers who say there is no alternative to their handy plastic packs.

It is evident that more research is needed into the recovery and reuse of various materials and into the cost of collecting and recycling containers as opposed to producing new ones. Unnecessary packaging, intended to be used just once, and make things look better so more people will buy them, is clearly becoming increasingly absurd. But it is not so much a question of doing away with packaging as using it sensibly. What is needed now is a more advanced approach to using scarce resources for what is, after all, a relatively unimportant function.

The ability of micro-organisms to find ways to evade the action of the drugs used to cure the infections they cause is increasingly recognized as a global public health issue. Some bacteria have developed mechanisms which make them resistant to many of the antibiotics normally used for their treatment (multi-drug resistant bacteria), so pose particular difficulties, as there may be few or no alternative options for therapy. They constitute a growing and global publlic health problem.

WHO suggests that countries should be prepared to implement hospital infection control measures to limit the spread of multi-drug resistant strains and to reinforce national policy on prudent use of antibiotics, reducing the generation of antibiotic resistant bacteria.

An article punished in The Lancet Infectious Diseases on 11 August 2010 identified a new gene that enables some types of bacteria to be highly resistant to almost all antibiotics. The article "has drawn attention to the issue of AMR (antimicrobial resistance) and, in particular, has raised awareness of infections caused by multidrug resistant bacteria.

While multi-drug resistant bacteria are not new and will continue to appear, this development requires monitoring and further study to understand the extent and modes of transmission, and to define the most effective measures for control.

Those called upon to be alert to the problem of antimicrobial resistance and take appropriate action include consumers, managers of hospitals, patients, as well as national govemments, the pharmaceutical industry and intemational agencies.

WHO strongly recommends that governments focus control and prevention efforts in the following areas like surveillance for antimicrobial resistance; rational antibiotic use, including education of healthcare workers and the public in the appropriate use of antibiotics; introducing or enforcing legislation related to stopping the selling of antibiotics without prescription; and strict adherence to infection prevention and control measures, including the use of hand-washing measures, particularly in healthcare facilities.

Successful control of multidrug-resistant microorganisms has been documented in many countries, and the existing and well-known infection prevention and control measures can effectively reduce transmission of multi-drug resistant organisms if systematically implemented.

WHO will continue to support countries to develop relevant policies, and to coordinate international efforts to combat antimicrobial resistance. Antimicrobial resistance will be the theme of WHO"s World Health Day 2011.

"Two types of breaches occurred, " Whetten-Goldstein noted. "The first was a more obvious type of breach. One example was a nurse who told her child that her patient was HIV-positive out of concern that her child would play with the patient&39;s child."

"The other type of breach was more subtle, one that providers might not consider breaches, " Whetten-Goldstein explained. "This type of breach involves providers talking about a patient&39;s HIV status without the patient&39;s knowledge of the interaction."

"The law allows the sharing of information between providers within the same institution, but patient&39;s consent must be obtained before providers at different institutions can share information,"she pointed out.

"Patients in the study wanted providers to tell them when they are going to share information with other providers and why it is being done," Whetten-Goldstein said. "They also felt that providers should be punished when a breach occurs."

"However, because patients are often reluctant to seek legal action which may further expose their status, they felt that the system should regulate itself," she added.

Who decides whether a patient is to die? This issue has not been solved legally in the United States.<br>

The matter is left to state law, which usually allows the physician in charge to suggest the option of death to a patient&39;s relatives, especially if the patient is brain dead. In an attempt to make decisions about when their own lives should end, several terminally ill patients in the early 1990s used a controversial suicide device, developed by Dr. Jack Kevorkian, to end their lives.<br>

In parts of Europe, the decision-making process has become very flexible. Even in cases where the patients are not brain dead, patients have been put to death without their approval at the request of relatives or at the suggestion of physicians. Many cases of passive euthanasia involve old people or newbom infants. The principle justifying this practice is that such individuals have a "life not worthy of life".<br>

In countries where passive euthanasia is not legal, the court systems have proved very tolerant in dealing with medical personnel who practice it. In Japan, for example, if physicians follow certain guidelines they may actively carry out mercy killings on hopelessly ill people.
